Male Testosterone
Total and free testosterone lower limits in men.

For the diagnosis of hypogonadism in men, total testosterone must be below 300 ng/dL (10.4 nmol/L) in at least 2 separate fasting morning measurements, along with clinical symptoms.
| Category | Total Testosterone (ng/dL) | Condition Analysis |
|---|---|---|
| Low (Hypogonadism) | < 300 | Clinical deficiency; SHBG and LH should be checked |
| Borderline (Gray Area) | 300 – 450 | Free testosterone is sought based on symptoms |
| Normal / Optimal | 450 – 850 | Physiologically healthy level |
| High | > 1000 | Exogenous supplementation or adrenal origin investigated |
Clinical Evaluation and Symptoms
In the process of evaluating low testosterone in men, laboratory results must always be assessed together with the patient's clinical picture, symptoms, and additional biomarkers. A single test result is not sufficient for a definitive diagnosis.
Which Medical Specialty Should Be Consulted?
To evaluate out-of-reference values in your results, consult the relevant specialist physician (Obstetrics and Gynecology, Internal Medicine, Endocrinology, or Pediatrics).
Does abdominal fat reduce testosterone?
Yes, the aromatase enzyme in visceral adipose tissue converts testosterone into estrogen, creating a vicious cycle.
My total testosterone is normal, but I have symptoms, why?
If SHBG protein is high, bioavailable free testosterone may remain low.
